Guidelines for completing the
request for insertion form —
Non-federal government clients

Sections 1, 2 and 3 are mandatory. Missing information might delay the publication of your notices.

Section 1: Client Information

Requesting organization’s name

Indicate the legal name of your company, group or association.

Address

Provide the complete mailing address of the contact person.

Contact name

Provide the name of the person whom we should contact to acknowledge receipt of the notice and for questions about the content, if necessary, etc.

Telephone and extension numbers

Indicate the telephone number and extension number (if applicable) of the contact name you provided.

Email

Please provide the email address of the contact person.

Section 2: Notices

Title of document submitted

Indicate clearly the title of your notice.

Date submitted

Indicate the submission date of your notice.

Published under the authority of (section of Act, Regulations, etc.)

Indicate the name of the act or regulations under which your notice must be published. Please note that all notices appearing in the Canada Gazette must be published in accordance with a federal act or regulations.

Date of publication expected

Indicate the expected date of publication. Refer to the Deadline Schedule for the list of publication dates and related deadlines. Some deadlines vary due to statutory holidays.

Section 3: Invoicing

Organization’s name

Indicate the legal name of your company, group or association.

Address

Provide the complete mailing address where invoices and a free copy of the Canada Gazette should be sent.

Contact name

Provide a contact name for all questions related to your invoice.

Telephone and extension numbers

Provide a telephone number and extension number (if applicable) of the person responsible for responding to questions about your invoice.

Email

Provide the email address of the contact who will answer all questions related to your invoice.

Do you need?

The Canada Gazette Directorate offers translation services, for a fee, for three types of notices: plans deposited, surrender of charter and relocation of head office. Contact us for further information on the costs associated with these services.

A legal affidavit, official proof of the publication of your notice, costs $22 and is available upon request.

Number of times published

Most notices need only be published once; however, some acts and regulations require that certain types of notices be published four times.

Text submitted on diskette, CD-ROM or USB Key

Save the English and French versions of your notice (Microsoft Word format only) as two separate files on a diskette, CD-ROM or USB key that is neither encrypted nor password protected. Notices saved on encrypted or password protected USB keys may not be processed and published on the required date. USB keys will be returned by mail to the contact person indicated in section 1.

Section 4: For use by the Canada Gazette Directorate

This portion will be completed by the Canada Gazette Directorate.


Please return your completed form, two copies of your notice and the Word version on a diskette, CD-ROM or USB key to the Canada Gazette Directorate, 350 Albert Street, 5th Floor, Ottawa, Ontario K1A 0S5.
